Specific Job DetailGeneral Responsibilities oAssesses emotional factors related to terminal illness.
oAssists the physician and other IDT/IDG members in recognizing and understanding the social/mental stressand/or disorder that exacerbates the symptoms related to terminal illness.
oAssesses the patient/family psychosocial status, potential for risk of suicide and/or abuse or neglect.
oAssesses environmental resources and obstacles to maintaining safety.
oParticipates in the development and revision of the plan of care.
Provides social services including:
oShort-term individual counselingoCrisis interventionoAssistance in providing information and preparation of advance directivesoFuneral planning issues and transfer of responsibilities regarding fiscal, legal and health care decisionsoPrepares clinical and progress notes.
oIdentifies family dynamics and communication patterns.
oInvolves the patient/family in the plan of care.
oIdentifies and utilizes appropriate community resources and assesses patient/family ability to access them.
oParticipates in in-service programs.
oEvaluates patient/family response to psychosocial interventions.
oAssesses caregiver's ability to function adequately.
oAssesses need for counseling related to risk assessment for pathological grief.
oAssesses special needs related to cultural diversity including communication, space, role of family members andspecial traditions.
oIdentifying the developmental level of patient/family and obstacles to learning or ability to participate in care ofpatient.
oAddresses patient/family questions and issues.
oIdentifies obstacles to compliance and assists in understanding goals of interventions.
oIdentifies support systems available to reduce stress and facilitate coping with end-of-life care.
oEvaluates for long-term care when appropriate and assesses ability to accept change in level of care.
oCommunicates psychosocial information to in patient facility when level of care is changed.
oAssists patient/family in assessing financial resources when appropriate.
oIdentifies patient/family needs when discharged or when level of care changes.
oEvaluates patient/family response to interventions when referred to community agency and satisfaction of theservice provided.
oAssesses bereavement needs.

Educational Requirements Masters Degree in Social Work from an accredited institution.
May require licensure or certificate as required by the state, and in good standing.
(if applicable) Position Requirements Prefer one year experience in a health care setting.
100% travel necessary on a daily basis.
Demonstrate problem solving and decision making skills.
Demonstrated ability to work will with an interdisciplinary team.
Excellent communication skills.
Ability to develop and maintain rapport with the client and family.
